Medicated Eyedrops

Glaucoma occurs when fluid builds up in the front of the eye and causes pressure inside the eye to build. In open-angle glaucoma, this happens slowly. In angle-closure glaucoma, this happens quickly.

The objective of glaucoma treatment is to improve the outflow of fluid from the eye and lower intraocular pressure. One way that we can do this is with the regular use of medicated eyedrops.

Some medicated eyedrops for glaucoma reduce the amount of fluid the eye produces; others help fluid drain from the eye better. Both types of eyedrops should be taken as directed by our doctors.

The type of eyedrop we recommend for your case will depend on factors such as other medications you take, as certain drops cause problems when taken with other medications. Sometimes we recommend using several eye drops in combination with each other.

Laser Surgery for Glaucoma

Normally, fluid produced in the eye filters out from the eye through the open angle located where the iris and cornea meet. At this angle, there is spongy meshwork similar to a drain, and the fluid flows through it to leave the eye.

Sometimes our San Diego glaucoma treatment team recommends laser glaucoma surgery, in which we use a laser to create several burns that stretch the drainage holes in this meshwork. This improves the outflow of fluid from the eye. This procedure, known as laser trabeculoplasty, can be performed in our office or at a local eye clinic. Normally each eye is treated separately, several days or several weeks apart.

Another laser surgery for glaucoma, known as laser iridotomy, involves using a laser to create tiny holes in the iris to improve the outflow of fluid.

Traditional Surgery

Historically eye doctors treated glaucoma with conventional surgery to remove tissue and create a new channel for fluid to drain from the eye. Depending on the specific procedure, either a small piece of the iris or a small piece of the drainage system can be removed. Eye doctors can also place shunts or stents to help improve the outflow of fluid from the eye.

Typically we recommend the use of medicated eye drops in conjunction with laser or traditional surgery.
